Medical Documentation

Students registering with Accessibility are required to provide current medical documentation showing evidence of their disability/medical condition or personal circumstances.

Documentation Requirements for Registering with Accessibility 

To register with Accessibility for support, including an EAP, you must provide medical documentation with a diagnosis, the date of onset and duration of required support (for short term conditions) and any recommendations to assist you with your studies.

This allows Accessibility to:

  • Provide an Education Access Plan with recommended reasonable adjustments for your studies at ANU
  • Support university staff to meet your needs in relation to your disability or health condition(s).
  • Ensure that you are protected by ANU policies and legislation.

Documentation other than the Health Practitioner Report is acceptable if:

  • It is completed by a relevant medical or allied health professional.
  • For short-term conditions (persists for less than 2 years), is current, includes the date of onset and duration of required support.
  • For long-term conditions (persists for 2 years plus) and disabilities, is no more than 5 years old.
  • Is in English (any translation must be NAATI certified).

 

Please note documents sent to the University Admissions Centre (UAC) or ANU Admissions Office with your university application are not forwarded to Accessibility. If your documentation is not sufficient, we will contact you to advise you to submit additional evidence of your disability/medical condition(s) or personal circumstances.
